Care Team

A named team, not a loose referral chain

TerraMD is built around role clarity. The coach does not replace the doctor. The doctor does not carry the entire implementation burden. The patient is not left to coordinate everything alone.

Team Roles

What each part of the team is responsible for

Clinical Integrative Health Coach (CIHC)

Provides continuity, implementation support, symptom tracking, and behavior change guidance.

  • Regular contact and accountability
  • Practical guidance around sleep, nutrition, breathing, movement, and stress regulation
  • Escalation when patient progress stalls or symptoms shift

Doctor / NP

Handles review, clinical decision-making, prescriptions, diagnostic reasoning, and red-flag escalation.

  • Interprets labs in context
  • Supports medication review and prescribing when appropriate
  • Decides when urgent evaluation or referral is needed

Urgent care escalation

Keeps TerraMD credible for acute needs instead of forcing patients into a disconnected emergency path.

  • Acute symptom triage
  • Immediate review for issues outside routine program flow
  • Referral or transfer when escalation is the right call

How They Work Together

Coordination is the actual product

Review

Shared visibility into the patient plan

WalkNorth keeps measurements, messages, milestones, and trend review visible so the whole system can respond faster.

Act

The CIHC keeps the plan moving between appointments

That means the patient is not left alone after the consult, and the doctor is not asked to carry the implementation burden alone.

Escalate

Medical oversight is available when the plan needs clinical authority

Red flags, prescriptions, acute issues, and meaningful clinical changes go back through the medical layer.

Why This Matters

Better outcomes depend on less fragmentation

Clear ownership

Patients do not need to guess who is responsible for continuity, escalation, or review.

Daily life translation

CIHC support turns good medical advice into something patients can actually carry into work, home, and travel.

Lower drop-off risk

Because the team is coordinated, fewer patients disappear after the first improvement or the first setback.
